    Sunset Neighborhood is a new “Community Concept” that
offers services for seniors that choose to “age in place” in their own homes.  It is a membership program that entitles the member to a professional and experienced Care Manager who assesses the senior’s needs in their home.  Based on the assessment, support services are recommended and arranged by the staff.  The Care Manager will follow-up to ensure the members are satisfied and the services are fulfilling the need.

    The Mission of Sunset Neighborhood holds a high regard for the “rights” of seniors.  The Care Managers are trained to be a strong advocate for our members, while keeping their independence as a priority, and maintaining their safety.  We have prescreened the vendors, and will only refer those vendors who hold the same high standard for our members. 

    The Care Managers are available to family members or friends to keep an open line of communication, regarding services that have been recommended and the overall well being of their loved one.
