Q: WHAT IS A CARE MANAGER?


A: Sunset Neighborhood Care Manager is a professionally trained nurse or social worker that has years of experience in the geriatric field of health-related and home care. They are trained to identify problems that many elderly face on a daily basis. They have the resources to match a service provider to meet the need or resolve a problem.


Q: WHAT WILL THE CARE MANAGER DO?


A: The Care Manager will visit the senior’s home as needed throughout the year. The Care Manager is trained to identify potential fall risks, nutritional concerns, personal hygiene issues, housekeeping, health care decisions, organizing medications, assess for mental health concerns, provide a large print emergency contact list of family, etc., address transportation issues, and help improve the socialization and much more.


Q: HOW DO I KNOW IF MY ELDERLY FAMILY MEMBER NEEDS THE SUNSET NEIGHBORHOOD PROGRAM?


A: If you are asking yourself any of the following questions, there is a good chance everyone will benefit from this program:

  • Is my parent safe at home?
  • Are their bills being paid in a timely manner?
  • Do I stay awake at night worrying about their well being?
  • How can I help my parents? I know they want to stay at home as long as possible.
  • What did they eat today?

Q: ARE MEMBERS OBLIGATED TO ACCEPT THE SERVICES FROM THE SERVICE PROVIDERS/VENDORS?


A: NO, members are not obligated to accept any service they do not want. Although if a Care Manager recommends a service to improve or enhance a member’s life, and it is refused, Sunset Neighborhood will not be held responsible for future problems related to the identified problem. The Care Managers will continue to monitor the member’s safety by personal home visits and keep the family informed of concerns.


Q: ARE THERE ELIGIBILITY REQUIREMENTS TO JOIN THE SUNSET NEIGHBORHOOD PROGRAM?


A: At this time, the requirements are to be at least 55 years old and a resident of the village of New Hartford and surrounding areas.

Q: CAN WE TRUST THE SERVICE PROVIDERS/VENDORS?


A: The service providers have been interviewed and pre-screened by the Sunset Neighborhood to ensure their reputation and honest work ethics. All providers have proper insurances and are in good standing with the Better Business Bureau. Although not all service providers will offer a discount, we encourage them to do so upon their signing up with the program.
